Did you take a animal companion or a domain for your natures bond?
HP should be 8 + con mod (+3)
your weapon damage adds your str mod
you didn't include druid saves (fort reflex and wis saves)
both your traits are magic traits, you can only have one of any type of trait (combat, faith, magic ...... etc)
list all your equipment and weights,
fill in your initiative and carrying capacity (loads and lifts)

top right corner, you need to put in your number of skills and saves that your druid level gives
you need to take skills, how many is whatever druid starts with + int modifier
